Prepare for Your Visit

Treating cancer is a highly sophisticated process; therefore we need a great deal of information. At the time of your first visit, you will have plenty of questions and we want to make sure we provide answers quickly and accurately. Following the steps below will help you get the most from your first visit, which in turn will help us develop the most effective treatment plan possible.

Be sure to bring your radiology films or CD's and have your New Patient Forms filled out, which will include your medical history (Please click here for our New Patient Forms). Be able to tell us all the medications you are currently taking. Also make sure to have your insurance information.

You will want to dress comfortably since it is likely that the first visit may take a while. We also encourage you to bring along a friend or loved one who may be able to help you make choices with your cancer team.

Please arrive 30 minutes before your appointment time on the first visit so that we can make sure all the necessary forms have been completed.
